This report sets out a comprehensive high-level approach for the establishment of nationally consistent foundational primary care dataset for New Zealand.

The Primary Health Data Set Programme (PHDP) partnered with the primary care sector to co-design the proposed approach. The overarching aim of PHDP is to improve health outcomes for New Zealanders by making a wide range of data from many sources more accessible.
