From the time someone is referred for specialist advice through to tests, surgery and follow ups, service users’ and health providers alike will have a clear, shared view of the health journey.
The health journey is the time from when you develop a condition needing treatment to the time the problem is either resolved or under control. This flow diagram outlines the basic steps in the Planned Care process:
