To create or upgrade your My Health Account Workforce, you can choose from one of several New Zealand or Australian identity documents to confirm who you are.

New Zealand driver licence

The driver licence details that you must enter in My Health Account Workforce are highlighted.

New Zealand passport

The New Zealand government introduced a new passport on 3 May 2021 (the RA series). An example is below, along with the previous version, which is still in circulation. The passport details that you must enter in My Health Account Workforce are highlighted.

New Zealand birth certificate issued or reissued since 1 January 2003

The New Zealand government introduced a new birth certificate on 1 January 2003. The birth certificate registration number is highlighted in the example below.

New Zealand birth certificate issued or reissued before 1 January 2003

For birth certificates issued or reissued before 1 January 2003, you must also enter the mother’s first, middle (if any), and last names and the other parent’s first, middle (if any), and last names.

New Zealand citizenship certificate

The citizenship certificate number that you must enter in My Health Account Workforce is highlighted in the example below.

Kiwi Access Card

Your Kiwi Access card must be less than 18 months old in order to use it for the identity document check. The details that you must enter in My Health Account Workforce are highlighted.

Australian passport

The Australian government launched a new passport (the R Series) on September 23, 2022. An example is below, along with one of the previous P Series, which is still in circulation. The details that you must enter in My Health Account Workforce are highlighted.

Australian birth certificate

From 2 August 2021, new security paper was introduced for Australian Birth Certificates from all states.

You must enter either the birth certificate Registration Number or the Certificate number. The format of these numbers varies from state to state.
